I hereby acknowledge and agree to the terms and conditions outlined in this Exercise Participation Waiver ("Waiver") in consideration for my participation in any group programs, community-led exercise events, exercise testing, innovative exercise-based program participation, individual appointments, and higher risk exercise participation conducted by Healthy Lifestyles Australia.
Assumption of Risk:
I understand that participation in physical activities involves inherent risks, including but not limited to, the risk of injury, illness, and the potential for aggravation of pre-existing health conditions. I acknowledge that the exercise programs and activities conducted by Healthy Lifestyles Australia may involve higher risk exercises that could result in injury.
Health and Fitness Certification:
I hereby certify that I am in good physical condition and have no medical or health-related issues that would prevent me from safely participating in the exercise programs offered by Healthy Lifestyles Australia. I agree to inform Healthy Lifestyles Australia of any changes to my health status that may impact my ability to participate in these programs.
Release of Liability:
I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby waive, release, and discharge Healthy Lifestyles Australia, its officers, employees, agents, and representatives from any and all claims, liabilities, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me during or as a result of my participation in the aforementioned activities.
Emergency Medical Treatment Authorization:
In the event of an emergency, I authorize Healthy Lifestyles Australia and its representatives to seek and consent to emergency medical treatment for me. I understand that efforts will be made to contact me or my emergency contact person prior to any medical treatment.
Photographic and Video Release:
I grant Healthy Lifestyles Australia the right to take photographs and videos of me during my participation in the activities. I understand that these images may be used for promotional and educational purposes, and I waive any right to compensation for such use.
Agreement to Follow Instructions:
I agree to follow all instructions provided by Healthy Lifestyles Australia staff during the programs and activities. I understand that failure to adhere to instructions may increase the risk of injury and may result in my removal from the activity.
By signing this Waiver, I acknowledge that I have read, understand, and voluntarily agree to all the terms and conditions outlined herein.
